Please be aware that our program relies on donations, and the availability of equipment depends on our stock. Some requests may be unfulfilled due to limited availability. We appreciate your understanding and patience as we work to meet the needs of our community.

This form is for individuals only. Each family member requesting equipment needs to fill out a separate form.

Processing Time

  • Requests typically take 5-10 business days to process, excluding holidays.
  • Larger requests for groups may take an additional 5-10 business days.

Pick Up & Shipping

  • You are responsible for arranging your own pick up and shipping. We will drop off equipment at a shipping location for out of town requests, but at this time we are unable to assist with the cost of shipping.
