Update Regarding Individual Requests for Hockey Equipment

Due to the high volume of requests we are currently receiving for hockey equipment, we have had to limit requests to those who are actively participating in a hockey program.

To request a full set of hockey equipment, you must be involved in an organized under-18 hockey program as a member of a team.
Skates, helmets, gloves, and sticks are available for those participating in an under-18 recreational/developmental skating program.

Please note that applications are open for all other types of sports equipment requests.

Individual Equipment Request Guidelines

One Form per Person: This form is for individual requests only.
- If you are filling out a request for multiple people in one family, each family member needing equipment must fill out a separate form.

Download and Save: You can also download the form to your computer and print it. Make sure to complete all required fields before saving.
Submit Your Request: Once you've filled out the form, submit it using one of the following methods:
1. Fax: 204-925-5716, ATTN Donna Matheison.
2. Mail: 321-145 Pacific Ave, Winnipeg MB R3B 2Z6.
3. Email: warehousesupervisor@masrc.com.

Reminders

Processing Time

Requests typically take 5-10 business days to process, excluding holidays.

Pick Up & Shipping

You are responsible for arranging your own pick up and shipping. We will drop off equipment at a shipping location for out of town requests, but at this time we are unable to assist with the cost of shipping.
